All on 4 dental implants

All on 4 (All on 6) is a modern solution for fully edentulous jaws that enables a fixed dental restoration supported by only four implants. The All on 6 version uses six implants instead. This approach is ideal for patients seeking a stable, long-term, and aesthetically pleasing alternative to traditional removable dentures.

What is important to know about All on 4 (All on 6)?

  • Dental implants

In traditional implant treatments, each tooth may require its own implant. However, with this method, only four dental implants are used to support the entire arch. These implants are strategically placed to provide solid support for a bridge or denture that replaces all teeth in the jaw.
  • Duration of all on 4 (all on 6) treatment

The procedure usually takes less time compared to conventional implant methods. This reduces the need for multiple surgical interventions and allows patients to return to normal life more quickly.
  • Stability and function

The All on 4 (All on 6) procedure ensures strong and stable support for the prosthesis, allowing patients to chew and speak normally. Additionally, the implants help preserve bone density in the jaw.
  • Maintenance and hygiene

All on 4 (All on 6) bridges and dentures are easy to maintain with regular brushing and cleaning. Thanks to the implant stability, patients don’t have to worry about slipping dentures or complicated cleaning routines.

Materials used in All on 4 (All on 6)

All on 4 (or All on 6) bridges or dentures are typically made from several materials to ensure the best balance of durability, function, and aesthetics. The key materials include:
  • Metal framework: The base structure of an All on 4 (or All on 6) bridge is made from metal, often titanium or similar alloys. The metal framework provides strength and stability to the restoration. Titanium is commonly used because it is biocompatible, meaning it integrates well with the oral tissues and minimizes the risk of rejection.
  • Ceramic or porcelain: The visible part of the bridge that replicates the look of natural teeth is made from ceramic or porcelain. These materials are chosen for their excellent aesthetics and ability to mimic the natural appearance of teeth. They are also stain-resistant and maintain their color over time.
  • Acrylic or other materials: Some All on 4 (All on 6) restorations may feature an acrylic base, which is used to recreate the appearance and feel of natural gum tissue.

Why choose All on 4 (All on 6) at Centrodent?

The All on 4 (All on 6) treatment concept is not suitable for every patient. Although this therapy is very popular, it delivers excellent results only in patients who meet specific clinical criteria. That’s why we recommend it exclusively to candidates who are ideal for this type of treatment.

  • Functionality and aesthetics

Our All on 4 (All on 6) restorations meet all functional and aesthetic standards. They are designed to allow easy maintenance and provide long-term durability.
  • Materials

We select the materials for All on 4 (All on 6) restorations based on each patient’s specific clinical needs. Not every patient is a suitable candidate for a zirconia-based All on 4 (All on 6), which is why the choice of material is always tailored to individual conditions.
  • Hygiene protocols

We apply the highest standards of sterilization, disinfection, and infection control to ensure a sterile environment and maximum safety during every surgical procedure.
  • Check-ups

Regular check-ups are essential after placing an All on 4 (All on 6) restoration. Without proper oral hygiene and follow-up care, the same issues that led to tooth loss can compromise the success of the All on 4 (All on 6) system. This is why we schedule routine follow-up appointments for all our patients.

All on 4 (All on 6) procedure overview

This stage includes a thorough clinical examination, X-rays, and a CT scan to assess bone condition and determine whether the patient is a suitable candidate for All on 4 (All on 6). Based on these findings, an individual treatment plan is created.

Four implants are surgically placed into the jawbone, after which a temporary prosthetic solution is attached. The patient wears the temporary All on 4 (All on 6) for three to five months, during which time the implants must osseointegrate - meaning they fuse with the bone.

After successful osseointegration, precise impressions are taken to create the final All on 4 (All on 6) prosthetic solution.

In this phase, the final All on 4 (All on 6) prosthetic is placed to replace the temporary one.

The first step involves removing the temporary prosthetic, followed by a thorough check of the implants and surrounding tissues to ensure all conditions are met for placing the permanent solution.

Next, the definitive All on 4 (All on 6) is carefully fitted. It is custom-made based on the impressions and tailored to perfectly match the patient’s needs. This marks the completion of the treatment process.

Common questions about All on 4 (All on 6)

The difference between porcelain and acrylic All on 4 mainly refers to the materials used to make the prosthesis. Both types are used in the All on 4 (All on 6) procedure to replace all teeth in one jaw, but the materials differ in their properties and performance.

Here’s a breakdown:

Porcelain All on 4 (All on 6):
  • Material: These bridges have a metal base (often titanium) covered with a top layer of porcelain. The metal base provides strength and stability, while porcelain mimics the color and appearance of natural teeth.
  • Advantages: Porcelain bridges offer superior aesthetics and look very natural. Porcelain resists staining and maintains its color over time. It's also a highly durable material.
  • Disadvantages: Porcelain restorations are generally more expensive than acrylic ones. If damaged or fractured, they often need to be remade.
Acrylic All on 4 (All on 6):
  • Material: These bridges are typically made with a metal base and an upper layer of acrylic (also known as acrylic resin). Acrylic is a polymer material.
  • Advantages: Acrylic bridges are usually more affordable than porcelain ones. They’re also lighter, which can reduce stress on the implants. Acrylic is easier to repair or adjust if any damage occurs.
  • Disadvantages: Acrylic bridges are not as natural-looking as porcelain ones, although modern materials have improved their aesthetics. Acrylic can also discolor more easily or get damaged over time.

The choice between porcelain and acrylic depends on your personal preferences, budget, and your dentist’s recommendations. It’s important to consult with your dentist to determine which material best suits your individual needs and goals.

While All on 4 (All on 6) therapy offers many benefits for full-arch tooth replacement, there are also some limitations that should be considered when deciding on this treatment.

Here are some potential downsides of the All on 4 (All on 6) procedure:
  • Cost: The procedure can be costly, especially if porcelain material is used for the prosthesis. The final price depends on the type of dental implants and the condition of the jawbone.
  • Surgical procedure: Implant placement requires oral surgery. Although it's less invasive than placing individual implants for each tooth, it still carries certain risks and involves a recovery period.
  • Recovery: After surgery, healing and adjustment time are required. Recovery may involve swelling, discomfort, and temporary changes in diet and physical activity.
  • Lack of individualization: Since one prosthesis replaces the entire arch, some patients may feel that their smile lacks uniqueness or individuality. This is a general limitation of all full-arch prosthetic solutions.
  • Maintenance: All on 4 (All on 6) bridges need to be cleaned thoroughly and regularly to prevent plaque and tartar buildup. Implant and prosthesis condition should be checked over time.
  • Not suitable for everyone: This treatment may not be appropriate for some patients, particularly those with certain health conditions or specific anatomical limitations.

It's important to consult a qualified dentist to assess whether All on 4 is the best option for your specific case. Both the advantages and disadvantages should be carefully weighed to make an informed decision about your oral health and future smile.

The cost of the All on 4 (All on 6) procedure can vary significantly depending on several factors. One of the main variables is the type of material used for the prosthesis, as well as whether any complications are present in your individual case.

Here are some general guidelines on pricing:
  • Dentist qualifications: Highly experienced and well-trained dentists or oral surgeons tend to charge more for their services. However, skilled professionals often deliver better long-term results and provide a higher level of safety throughout the treatment.
  • Prosthetic materials: The choice of material affects the cost. Porcelain prostheses are usually more expensive than acrylic ones but offer superior aesthetics and durability.
  • Complications and additional treatments: In some cases, additional treatments may be necessary, such as extractions, bone grafting, ridge leveling, or infection treatment, which can raise the total cost.
The price difference between All on 4 and All on 6 systems also depends on the number of implants, as All on 4 uses 4 implants while All on 6 involves 6.

To receive an accurate cost estimate for the All on 4 (All on 6) treatment, we recommend a consultation with our dental team. After a clinical examination and assessment of your general health and diagnostic results, they can provide a precise calculation based on your specific circumstances.

Whether you are a candidate for this procedure depends on your overall health, anatomical characteristics, and personal goals. Your dentist will need to carry out a thorough assessment to determine if All on 4 (All on 6) is suitable for you.

Key factors to consider when evaluating candidacy for All on 4 (All on 6):
  • Condition of your teeth and gums: This treatment is often ideal for individuals with multiple missing, damaged, or failing teeth. If your remaining teeth are not sustainable, or if you're missing a large number of teeth, this may be a strong indication for this type of prosthetic solution.
  • Jawbone health: To qualify for All on 4 (All on 6), your jawbone must be strong and deep enough to support dental implants. If bone density is insufficient, additional procedures like bone grafting may be required.
  • General health: Your overall health is crucial. People with serious health issues such as uncontrolled diabetes, heart conditions, or weakened immune systems may not be suitable candidates for surgical procedures.
  • Realistic expectations: It’s important to have a clear understanding of what this treatment can deliver. Talk to your dentist about your expectations regarding aesthetics, function, and longevity of the prosthesis to ensure they are realistic.
  • Financial considerations: All on 4 (All on 6) can be a significant investment, so it’s important to assess your budget and look into potential insurance coverage or financing options.

After a detailed assessment, your dentist will be able to determine whether implants and the All on 4 (All on 6) approach are right for you. If not, other options for tooth replacement such as traditional bridges, dentures, or individual implants may be considered. The key is working with an experienced dentist who can help you explore the best treatment based on your specific needs.

In the All on 4 system the entire dental restoration is supported by four implants, while in the all on 6 six implants are placed, providing more stable support and better distribution of chewing forces.

It is not advisable to place a zirconia All on 4 because four implants are not sufficient support for such a heavy prosthetic construction.

For a zirconia prosthetic restoration at least anAall on 6, meaning six implants, is required.

All on 4 is suitable when making an acrylic prosthetic restoration because this is a much lighter material that can be safely supported by four implants.

The final decision is always made after a detailed analysis of the bone and the individual needs of the patient, in order to achieve a long lasting and functional result.

It is particularly important for patients to know whether a zirconia All on 4 or All on 6 is being made as monolithic crowns or as zirconia layered with ceramic.

Since a prosthetic restoration made entirely of zirconia cannot meet aesthetic requirements to the same extent as a zirconia ceramic prosthetic restoration, it is crucial that patients are informed in advance about which type of prosthetic work will be placed.
