Article 1.

Subject of General Terms and Conditions
 
1.1. These General Terms and Conditions (hereinafter: Terms) govern the relationship between the Service Provider - Centrodent d.o.o., located at Tometići 1/b, 51215 Kastav, OIB: 69189510158 - as the provider of dental services, and the client in need of dental services (hereinafter: the Client). All mutual rights and obligations not explicitly governed by these Terms shall be subject to the applicable regulations of the Republic of Croatia.
1.2. By signing the Offer, the Client accepts the terms of use of the Service Provider as defined in these Terms. Upon submitting the Offer, the Service Provider shall provide the Client with these General Terms and Conditions and the Price List of dental services (hereinafter: the Price List).
 

Article 2.

Services provided by the Service Provider and service quality standards
 
2.1. The Service Provider offers dental services in accordance with professional medical regulations and the Act on Dental Medicine (Official Gazette NN 121/03, 117/08, 120/09, 46/21) as well as the Consumer Protection Act (NN 19/22, 59/23).
2.2. The Service Provider is obligated to present every service to the Client in a complete, clear, accurate and non-deceptive way, understandable to the average user, including information about potential deviations depending on the condition of each Client’s teeth.
2.3. Within its technical capabilities, the Service Provider is obligated to ensure equal access to its services for persons with disabilities.
 

Article 3.

Conditions for providing dental services
 
3.1. Dental services may be used by adult natural persons. For minors, parental or guardian consent is required.
3.2. Upon request for dental services, the Client must present their ID card to the Service Provider (and agrees to copies being taken), as needed to verify the Client in accordance with applicable legal requirements.
3.3. The Client agrees that the Service Provider may verify their identity, legal status, and financial capability through authorized bodies and with the Client’s explicit consent.
3.4. By signing the Questionnaire, the Client (a natural person) gives consent for the Service Provider to collect, store, and process their personal data, including OIB, for internal administrative purposes. The Client also consents to the Service Provider forwarding their data to financial institutions, banks, debt collection agencies and similar, in accordance with the Law on Personal Data Protection and for lawful purposes such as creditworthiness checks, payment assurance, and debt collection.
3.5. The end user agrees and gives consent for the Service Provider to process necessary data for the purpose of:
a) providing requested services;
b) offering additional related services;
c) handling complaints and ensuring service quality.
3.6. The Questionnaire contains: Client data (natural person who signed it and is financially responsible), details on used services, issued invoices, and the Client’s OIB.
3.7. The Client is obligated to accurately disclose any existing illnesses or allergies. The Service Provider cannot be held liable for complications resulting from undisclosed medical history.
 

Article 4.

Price list and payment terms
 
4.1. Dental service prices and all other fees are defined in the valid Price List at the time of service usage. The Service Provider reserves the right to amend the Price List and will ensure it is publicly available. The Price List is available on-site at the Service Provider’s premises.
4.2. In case of a Price List change, the Service Provider commits to honoring the previously accepted Offer (including a 15% deposit), provided it was signed and paid within the required period, even if new prices are less favorable for the Client.
4.3. The Service Provider is obligated to issue an invoice to all Clients clearly stating the services performed and indicating any applicable price adjustments or agreed reductions.
4.4. The Client agrees to pay for all services provided by the Service Provider upon completion.
4.5. On the day of impression taking, the Client is required to pay 50% of the total cost for the prosthetic work. If the Client fails to do so, the Service Provider reserves the right not to begin production. The remaining amount must be paid before or at the time of prosthetic delivery.
4.6. The Client is required to collect the prosthetic work within 1 (one) month of impression taking. Otherwise, the Service Provider may issue an invoice regardless of whether the work suits the Client, and the Client agrees to pay for it.
 

Article 5.

Refusal of service
 
5.1. The Service Provider reserves the right to refuse service in the following cases:
a) if the Client is a minor and does not have valid legal guardian consent;
b) if there is doubt about the identity, legal capacity, or authorization of the Client or their representative;
c) if the Client is financially incapable of paying;
d) if the Questionnaire is not completed or signed;
e) if the Client, in the Service Provider’s reasonable judgment, will not be able to fulfill their financial obligations;
f) if required payment security is not provided;
g) if there are outstanding debts to the Service Provider;
h) if prior agreements were violated by the Client;
i) if there is a medically justified reason not to perform a dental procedure, in which case the Client will receive an explanation of risks involved.
j) the Service Provider reserves the right to decline the provision of certain dental services if there is a valid medical reason to withhold the treatment. The Client will be clearly informed about the potential health consequences of such a decision.
